****ALERT****

Please do not send tournament applications to the District 1 Tournament Coordinator using registered mail or other mail that requires a signature upon receipt. This form of delivery is an unnecessary expense and may cause delay in delivery if no one is available to sign. You may choose to enclose a self-addressed/stamped post card with your application to verify receipt. Your District Tournament Coordinator is as close as the nearest mailbox!

Application Procedures for CYSA Tournaments

  1. Follow the tournament application procedures in the CYSA Team Manual. The application package and the CYSA Tournament calendar are available at www.cysanorth.org
  2. District 1 teams must submit a copy of the official CYSA team roster (goldenrod) with the application.
  3. Teams are not authorized to forward an application or check directly to a tournament. You must apply through the District 1 Tournament Coordinator.
  4. Tournament Applications must be submitted to the District 1 Tournament Coordinator by mail. Please send applications to:

    Patricia Flores
    District 1 Tournament Coordinator
    729 Shotwell Street
    San Francisco, CA 94110

If these application procedures are not met, your application will not be approved and it will be returned to you. Applications to CYSA tournaments in the new tournament calendar year will not be approved before May 1.

The District Tournament Coordinator will confirm the team is applying to the appropriate level tournament (i.e. Class 1 team to Class 1 tournament). See the Tournament calendar in the CYSA Team Manual for class designations.

GUEST PLAYERS: CYSA tournaments accepting guest players are indicated in the CYSA Tournament schedule. Please check the Cal-North website for policies regarding guest players.

Tips for Quick Processing of Your Application

Fill out the application completely with players' names in alphabetical order by last name, birth dates, gender, age, class, contact person's address and phone number, as well as an alternate contact.

A team official's signature must appear at the bottom.

Be sure to print and read the page.

Do not use staples.

You may send multiple tournament applications together in a 9" x 12" manila envelope. However use a large paper clip or small binder clip for each tournament application package.

Enclose two (2) copies of the tournament application (original and one copy for District 1 file).

Attach the check payable to the tournament to the original application with a paper clip. Do not staple. Do not send the check to the tournament: they cannot hold a spot without the tournament stamped application.

Address a standard-size envelope with the name of the tournament to which you are applying. Include your return address and sufficient postage.

Attach the (2) copies of each application, the check for the tournament and the addressed envelope with one paper clip. Please do not staple.

If you have been rejected for a tournament and you are applying for that same weekend, you must include a copy of the rejection letter with the new application.

Optional: Include a self-addressed, stamped postcard if you want confirmation that the tournament has received your application.

The less unfolding, un-stapling and sorting that has to be done, the faster your tournament packages can be processed.

All tournament applications will be sent out first-class mail only.

Please do not send your applications to us asking for a return receipt. If we are not at home, and have to go to the post office to pick up any mail, this can will delay your application being processed.

We cannot process faxed applications.

Please do not put the applications in my home mailbox. If the postal carrier comes after you drop off your application, they might pick up the your package.

Please make sure that the correct amount of postage is on the envelope or it will be rejected.

If there are any questions or problems, send email to D1TC@yahoo.com or call Patty Flores at (415) 641-7515.

Failure to comply will result in delays and you may not get into the tournaments to which you apply.

Out of State or Out of USA Travel

To travel to a tournament outside CYSA jurisdiction (including CYSA-South) you must complete travel forms and get them approved. Travel forms are available from the District Commissioner or the District Tournament Coordinator. The District Commissioner must receive all of the following material for processing.

  1. Completed "US YOUTH SOCCER APPLICATION TO TRAVEL" (multipage form).
  2. Completed "CYSA TRAVEL ROSTER" (BLUE FORM) the teams coach and their home league official must sign the form first. The approval stamp of the District Tournament Coordinator must be obtained before sending the form to the District Commissioner. To add players to a previously approved CYSA Travel Roster, you must fill out an additional CYSA Travel Roster and get all proper signatures.
  3. A copy of the "US Youth Soccer Application to Host a Tournament or Games" form for the competition in which you wish to participate. This is proof of US Youth Soccer sanction.
  4. A $10.00 check payable to CYSA District One. A check for late fees as listed on back of application.
  5. Two addressed envelopes with postage; one addressed to the tournament, and one self addressed envelope. Mail all forms to:

    Mary K. Miller
    109 Beaumont Ave.
    San Francisco, CA 94118

All paperwork will be forwarded to the CYSA state office for final approvals.

GUEST PLAYERS: Please see the CYSA website for guest player rules and regulations.

The above material must be submitted to the District Commissioner at least 30 days prior to travel within the United States or 90 day prior to travel outside the United States. In some instances a team may be notified of acceptance to a tournament after the filing deadline. The team must then present an official acceptance letter stating the acceptance date in order to avoid late fees. Penalty for late submission is $25.00; check payable to CYSA District 1.

Other Travel and Games

Any team desiring to travel outside the District 1 boundaries to play in a jamboree or informal games must notify the District Commissioner prior to the games. Without approval, CYSA insurance is not in force.

Unaffiliated tournaments or games:

Any team desiring to play games with non-USYS (unaffiliated) teams/leagues/tournaments must have the approval of the CYSA Board of Directors. AYSO, US Club, schools, Rec & Park leagues are examples of unaffiliated organizations. Please see the CYSA website for further details.
